Case Management and Supportive Services are keystones to assisting our clients reach the pathways to a stable and productive quality of life. Another critical service element is our Housing Placement Assistance (HPA) program. The HPA answers the question of how can anyone pursue treatment and stability when their housing situation is unstable or there is a threat housing stability.

 

Case Management Program Philosophy

 

An important driver of our case management program is its philosophy. For the person infected/affected with HIV/AIDS, self-empowerment and stability are the cornerstone to maintaining a healthy body, mind and lifestyle. HCC believes in supporting the client by bridging the gap between his/her needs and the resources that can be made available to meet those needs. Client should be empowered in order to access the benefits that he/she and his/her family are entitled to. This will permit him/her to take the necessary steps to improve their quality of life.

 

At HCC we are able to provide services to clients regardless of legal status. Our program is client-centered and views the client’s needs as a part of a whole person and not just one isolated issue in a vacuum. Our case managers will also passionately, yet professionally help clients and advocate for them for every issue affecting their well being.

 

Case Management Program Goals

 

The goals of HCC's Case Management Services are to:

 

1.     Develop and maintain a comprehensive plan towards stability for Haitian and other immigrants through advocacy and referrals to services for which they would otherwise not have access. 

 

2.     Collaborate with the client's identified family, significant others, support systems, and other agencies involved in his or her care to ensure the best possible service provision.

 

3.    Provide appropriate housing, medical, legal, entitlement benefits, education, nutrition, immigration, medical referrals and emotional/psychological support for the client. HCC will assist clients in developing and using social networks.

 

4.    Help clients with HIV/AIDS to function independently in society, as well as empower and educate them on discrimination and confidentiality issues to ensure that their rights are not violated.

 

Case Management/Supportive Services

 

Our Case Managers provide intensive case management services to HIV infected individuals of Haitian and/or Caribbean descent and their families. They act as a bridge between the client and the non-Haitian providers, providing translations to both parties. Case managers help the clients navigate the medical and social service systems and work to assure that service providers are educated and sensitized to the client's cultural beliefs and needs.
