CONGREGATE SUPPORTIVE HOUSING
Housing
1 and 2 Bedroom Family Units designed for PLWHA and their families. Some clients also receive home health aid services as well as visiting nurse services. Apartments are provided well-furnished upon admission. Client pays 30% programmatic rent fees if they work on receiving SSI’s, otherwise the programmatic fees are waived if clients do not receive any income.
Case Management
Case management establishes linkages with all providers and caregivers involved in client’s care, and helps clients deal with issues such as: enticement benefits, immigration, legal issues, escorts, advocacy, and translation when necessary.
Recreational Counseling
Recreational services are provided in order to create or encourage relaxation activities and groups in order to alleviate stress and tension.
Nutritionist Services
Nutritionist services are provided by a qualified dietician who provides nutritional support to clients in need. Food pyramid workshops are provided, where clients learn how to eat when sick, and understand caloric content of foods.
Tutoring
Our tutoring service provides help to children and adults who need academic support. ESL classes are provided to Haitian Creole and Spanish speaking individuals.
