HIV TESTING
HCC is happy to offer free, confidential rapid HIV testing screening services Monday-Friday at our community office location at 3807 Church Avenue, Brooklyn, NY 11203 (between E. 38th & E. 39th Sts.)
Our services are provided in a supportive, non-judgmental environment to assist you in identifying your HIV risks and risks for other sexually transmitted infections. You and your counselor work together to create a realistic plan to help reduce your risk for HIV and other STIs, and to promote your overall health
and well-being.
If you test positive for the HIV antibody, you'll have the opportunity to meet with our Linkage to Care specialist, who can help you with accessing medical care, disclosing your status to partners and loved ones, discussing safer sex options, treatment adherence, and much more. During this session, the specialist will help you identify your emotional and concrete needs and give you referrals for HCC services and our other partners in the community.
Our staff will also provide you with appropriate information and a referral for free treatment at our partner clinic.

LINKAGE TO CARE
Receiving an HIV diagnosis is a life changing event. Although we know that advances in medication and treatment have dramatically improved health outcomes for people living with HIV/AIDS, receiving this news can be a frightening, confusing and emotional experience. We understand this, and we know that getting a newly diagnosed person connected to medical care as early as possible is the best way to ensure their long-term health and well-being.
HCC’s Linkage to Care Program is designed to support clients through every step of this process, to ensure they understand the implications of an HIV diagnosis, and can access the many resources and supports that are available to them.
When a person tests positive for HIV at our Church Avenue site or in our Mobile Health Unit, our Prevention Navigator follows up and schedules a conversation. Their role is to learn more about the person -- what their circumstances are, what kind of support system theyhave in place, how they are feeling -- and to sharewith them any appropriate resources and recommendations, based on everything they’ve discussed.
Regardless of your income, class or insurance status, we can help connect you to the care and resources that are most relevant and helpful for you. Many clientsreceive care at our offices at 3807 Church Avenue in Brooklyn, or take advantage of the one-on-one counseling and support offered by HCC.
Others are referred to outside providers and resources. Some clients are matched with a medical case manager at HCC or a referral agency, so they can receive ongoing support with care and insurance, prescription drug access, or other issues.
Even if you did not get tested at HCC, but would like to speak with us about your needs and options for HIV care and support, we are happy to speak with you. If you’ve fallen out of medical care and would like to get reconnected, our staff will work with you and help get you back into care.
